Health Reset Program terms and conditions
Astute Simplicity Health members are covered under the St Lukes Health Reset Terms and Conditions, from the underwriter, St Lukes.
1. You may be eligible for the Health Reset Program, presented in partnership between the underwriter St Lukes, for Astute Simplicity Health and Salveo Healthcare (The Program) if you meet the inclusion/exclusion criteria of The Program.
2. Submitting your interest in The Program, does not guarantee that you are eligible for The Program or guarantee you a place in The Program.
3. You must obtain a referral or health summary from a General Practitioner at your own cost. The referral must confirm you are diagnosed with or at risk of a chronic disease, address the eligibility criteria, and be provided to Salveo Healthcare at the time of your initial consult.
4. The Program has limited capacity. Final eligibility for formal enrolment is determined by the underwriter of Astute Simplicity Health, St Lukes and Salveo Healthcare
5. If you are formally enrolled in The Program, your place in The Program is funded by the underwriter for Astute Simplicity Health, St Lukes. You are responsible for meeting all other costs associated with The Program, including (but not limited to) your internet and mobile phone costs, clothing, footwear, food, drink etc.
6. You consent to providing feedback for quality assurance and improvement during and after your participation in the program
7. To be eligible, members must hold any level of Hospital cover and have held cover with Astute Simplicity Health for more than 12 months.
8. Members must be financial for the entire duration of their participation in The Program.
Third Party Service Provider
9. The Program will be provided by Salveo Healthcare and if you are formally enrolled in The Program, you will receive services from Salveo Healthcare as part of The Program.
10. Salveo Healthcare can cease to provide The Program if Salveo Healthcare and/or the underwriter for Astute Simplicity Health, St Lukes decide, in their own discretion, that The Program is not assisting the participant.
11. Salveo Healthcare can cease to provide The Program if Salveo Healthcare and/or the underwriter for Astute Simplicity Health, St Lukes decide, in their own discretion the participant is not participating in The Program as is intended
12. St Lukes is not liable to you for any injury, loss or damage whatsoever or howsoever caused or arising directly or indirectly in connection with The Program and the services or information supplied either by the underwriter for Astute Simplicity Health, St Lukes or Salveo Healthcare in connection with The Program, except to the extent that such liability may not be lawfully limited or excluded.
13. Salveo operates 7am-7pm Weekdays and will only service members during this time.
14. In-person appointments are limited by geographical location as determined by Salveo Healthcare. Members residing outside of Tasmania will be serviced by telehealth in all instances.
15. Eligibility for physiotherapy and dietitian supports is determined by Salveo Healthcare nurses. Access to these funded sessions are limited.
Privacy
16. Astute Simplicity Health will collect, store, and use personal information about you in accordance with St Lukes’ Privacy Policy, which is available on our website.
17. Salveo Healthcare will collect, store, and use personal information about you in accordance with Salveo Healthcare’s Privacy Policy, which is available on their website.
18. You authorise your Medical Practitioner to share personal information about you, including health information, with the underwriter for Astute Simplicity Health, St Lukes and Salveo Healthcare in connection with The Program.
