To be eligible, Participants must: 

• Be 18 years of age or over;
• Currently hold an Astute Simplicity Health Hospital Product and are financial.
• Provide explicit written or verbal informed consent to participate in the program
• Be likely to respond to, and are able to actively participate in the program;
• Is not excluded by one or more of the program exclusions; and
• Meets the following clinical inclusion criteria:
  • Has a diagnosis of a chronic disease and able to provide evidence of this via a health summary or referral from a registered medical provider.
  • Is clinically stable as determined by Salveo Healthcare in an initial assessment.

Program Exclusion Criteria 

Program exclusions include any of the following relating to a Participant as reasonably determined by Salveo Healthcare, being (subject to change at the discretion of Salveo Healthcare) a Participant that: 

• Does not meet the Inclusion Criteria above;
• Will not benefit from participating in the Program;
• Is unable to actively participate in the Services due to barriers including, but not limited to:
  • Participant has technical challenges with engaging with the Clinitouch application or alternatives provided.
  • Participant experiences one or more condition/s or social circumstances that impact their ability to actively participate in the Program.
  • Is clinically unstable.
