Health Reset Program
Have you been newly diagnosed, recently hospitalised, or want to better understand your chronic condition?
The Health Reset Program (through the underwriter St Lukes and delivered in partnership with Salveo Healthcare) can give you the tools and support you need to reach your health goals.
What does the program involve?
The program will complement your current health care and support you by making an action plan to help you feel stronger, more confident, and more in control of your chronic condition.
The Health Reset Program (through the underwriter St Lukes) includes:
- Up to 3 months of one-on-one coaching with experienced Salveo Healthcare nurses.
- Options for tailored support from a dietitian or physiotherapist based on your personal health goals*.
- Set goals that matter to you and build a plan to reach them.
- Track your progress with the Clinitouch Connect App.
- Choose how you connect: in person or online, whatever works for your circumstances^.
*Eligibility for physiotherapy and dietician supports are determined by Salveo Healthcare nurses. Sessions are limited.
^In person appointments are limited by geographical location as determined by Salveo Healthcare. Members outside of Tasmania will be serviced by telehealth in all instances.
Am I eligible?
You may be eligible if:
- You currently hold Astute Simplicity Health Hospital cover and are financial
- You have been a member for a minimum of 12 months
- You have a compatible smart phone and reliable internet connection
- You have an eligible chronic condition and able to provide evidence of this from a healthcare professional
- You meet the clinical inclusion criteria after assessment by a Salveo Healthcare clinician
